Jury Service Response Form
Persons summoned for jury service in the Tyler Municipal Court may use this online form in lieu of returning the form on the reverse side of the summons received.

No person is excused from jury service until receiving confirmation of receipt of your request from the court and that the request to be excused has been approved by the judge.  Failing to appear for jury service is punishable by contempt of court.

If there is any other reason that you believe requires you to be excused from jury service, please explain.  Please bear in mind that business excuses are not valid excuses.

Terms & Conditions of Use:

1.  No person is excused from jury service upon the submission of this Response Form.  You must receive a telephone call or email from the court indicating that you are excused from appearing.

2.  Using this service less that 48 hours prior to your appearance date may not allow adequate time for court personnel to contact you.  This especially applies if your appearance date is on a Monday and you use this service on Friday.  If you do not receive notification that you have been excused, you must appear as ordered and present your excuse in person.

3.  All excuses are subject to verification.  Supporting documentation (school id, travel itineraries, doctor's notes) should be faxed to 903.531.1369.  Please include your name on any information submitted.

4.  Make certain that you are excused from jury service before you decide not to show.  Ignoring a jury summons is punishable by contempt of court which includes a $100.00 fine.
