All persons who are subject to the jurisdiction of the Municipal Court (i.e. those on a payment plan, waiting for trial or who otherwise have open cases in the court), are REQUIRED to keep the court informed of your current contact information. Failure to to keep this information up to date may result in the termination of your payment plan, additional charges of "failure to appear" being filed against you should you miss a court date, and a warrant being issued for your arrest. All information requested below is required to be submitted each time this form is completed regardless of whether or not there is a change in that information. However, you may place "NONE" in a box if you do not have that type of information (i.e., if you do not have a home telephone number or email address).
